The only event not seeming to work for me is Combat Event: I died. Anytime I die nothing is spoken, all the settings are said to say, I also typed /s in all the boxes, still nothing.
Just set I died to on and say and nothing happens.

I have marked this ticket as "declined" as a mix between "working as intended" and "the requested feature is impossible to implement or improve"
The problem here is that you can't speak in the /say or /yell channel while you're dead. It's Blizzard's game rule. However, you can use all of the other chat channels: party, raid, battleground, whispers, and the special self-only chat options in SpeakinSpell including the mysterious voice. Off the top of my head, I'm not sure about the emote channel, which may or may not be allowed while you're dead, but I assume it's probably not allowed, like /say.
The combat event in SpeakinSpell is working correctly, but it announces your death after you die, so in order to use it effectively, you must select a valid chat channel that can be used while you're dead, such as the party (/p) or raid (/ra) channels.
Blizzard does not provide the necessary event hooks and back-end support to change SpeakinSpell's event trigger for "Combat Event: I died" to fire immediately before you die while you can still use the /say channel. There is no prior alert that you're dying or about to die, so that by the time Blizzard alerts SpeakinSpell that you have died, your character is already completely dead, and thus unable to /say anything.
The closest we could come to announcing that you're almost dead, before you lose access to the /say channel because you're already completely dead, would have to be triggered by a low health limit, which is an open feature request in ticket 134: http://www.wowace.com/addons/speakinspell/tickets/134-low-health-and-mana/
