Bugfix for connected realms

Fylwind:
    - Deal with connected realms
    - fix the bug with connected realms
      - clarify which are presence IDs and which are toon IDs,
        although SendMessage works with both apparently
      - some minor cleanup
    - Indicate that <command> is case-sensitive
