Since the last patch i have noticed sometimes when you hit a keyboard stroke for an action bar you have to hit it twice. I tried keybinds and nonkey binds both had issues, its not everytime but id say 3/5 clicks. I also clicked but clicking didn't have any issues that way. I disabled every addon and put one on each time and bartender4 was the only one that maintained this issue. I've spoken to multiple players that are having the same results/issues. I have also tried 3 keyboards and 4 different computers. Thank you for looking into this or telling me a solution!
