7 - Not detecting at least one quest "item" properly
The "Area 52 Special" item needed for a quest doesn't show up in QuestItemBar (its for dropping bombs during a quest and/or its followup; info here: http://www.wowhead.com/?item=28132#provided-for ).
Its not listed as a "Quest Item" in its tooltip, which is likely causing a problem with your detect methods. There are likely more than a few items like that in-game, though (used for a quest, but not listed as a "quest item"). So you may want to keep an in-mod database for such items.
